1. Exception management

Log the event, quantify the exposure, assign one accountable owner, correct the current issue, then design a control that reduces recurrence.

Every material workflow should answer: what can fail, how would we know, who owns the control, what evidence exists, and what happens when it fails?

Framework

Exception management in five steps

  1. Log the exception. Capture what happened, when, severity, owner, status and measurable exposure.
  2. Quantify the exposure. Estimate direct loss, revenue at risk, overpayment, underbilling, rework or labor waste.
  3. Separate correction from prevention. Corrective action resolves this event; a control reduces the next one.
  4. Review by severity and age. Start weekly reviews with high-severity exceptions, overdue actions, open dollar exposure and recurring root causes.
  5. Automate after the control is understood. Define the trigger, expected output, tolerance, evidence and escalation path first.

Operational controls checklist

Weak-control warning: “someone checks it” is not a strong control if the owner, evidence, frequency, tolerance and escalation path are undefined.
